The point of this template is to make goal setting as effective and easy as possible. You're working backwards, starting at the place you want to end up in and finishing by setting daily habits that will help you get there.
Here is a more in-depth explanation of how it works:
The first section is a descriptive journaling part to help you envision your new self by the end of the year/whatever period you choose to do it in. It has 6 categories which can of course be tailored to your life and where you need to see change. Once you've described in detail what you want to look and feel like at the end of this, you pick one goal per category. Then you think of how you can get there, what action you can do every day and/or once a week to make the goal real - these are your new habits. From there you can put everything into one database for quick and easy access (which is what you need if you want to succeed). The last part is a space for you to paste a vision board if you're into that sort of thing.
